I am not Heather but corporate has never asked me for any forms. I
always have them fill out the form unless I have not other choice. I
like having something in writing showing what they ordered and all their
contact info. I do, however, think it is important that you sharpie over
the SS or enter the info sans SS somewhere else and shred or burn the form.
