A lot men do not know much about female anatomy and some have really poor skills because they were not educated properly. As a result they can physically hurt a women and thus cause her to clam up even more.
Women need to be very open about their own bodies enough to share with their husbands what kinds of touch is comfortable and stop them immediately when it is not and trust that their husband will be mature enough to not have their pride hurt.
