children, both of which I nursed. I had plugged milk duct a couple
times with each, and the simplest solution was to switch positions. So
if you are nursing using the cradle hold position, try the football
hold. The way the baby latches is a bit different then the cradle
hold, and it "drains" the milk duct differently. My mom gave me that
advice and it worked within two feedings. And for the engorement,
pump! Hope it helps!

At 4 days postpartum, you are experiencing not just milk volume, but swelling and some degree of inflammation with the milk supply getting started. The maternity book recommends citrus bliss, but as I recall, he would have used others. You may also want to try some cypress to help circulation or lemon for swelling and frankincense would reduce inflammation as well as ice packs 10-15 min. around the bottom/sides of the breasts every 1-2 hours. Make sure you have a good breast pump to soften the breast prior to latching baby on if you're too full . This helps get the milk supply flowing some and also everts the nipple so baby can find it easier. Try to offer every 2-3 hours and express if you are uncomfortably full. You can point the baby's nose or chin toward the plugged area and use some massage while baby is nursing . You may also want to look up "reverse pressure softening" on a search engine and see if that technique helps you out. It has been tremendously helpful to get baby latched during this time. Lavender and basil would help a milk supply and both have pain relieving qualities to them. Roman Chamomile is very soothing to tissue and relaxes , which may help with a milk letdown. Let me know if this helps you-I am somewhat new at recommending them for these issues, but my heart goes out to you and I love these oils!
